Hospice Wellington is offering a workshop on Advance Care Planning at the Main Library.
Do you know who will make decisions for you if you are unable to speak for yourself? This question is crucial for ensuring that if you are ever unable to speak for yourself due to illness or injury, someone you trust can make decisions in alignment with your values, beliefs and preferences.
The person who can make decisions for you in the event that you are unable to do so is a substitute decision-maker (SDM) or Power of attorney (POA). This is someone you have chosen or who has been legally designated to act on your behalf when it comes to healthcare decisions.
Having a conversation with your SDM about your health care wishes is essential to ensure they understand your values, beliefs and preferences. It's important to be clear about things like:
